Our team utilizes Interim Stabilization Therapy (IST) and the Silver Modified Atraumatic Restorative Technique (SMART) as minimally invasive approaches to managing suspected dental decay in residents living in long-term care settings. These approaches are particularly beneficial for individuals who may be medically fragile, have cognitive impairment, experience difficulty tolerating traditional dental treatment, or are unable to access community dental services. SMART combines the use of Silver Diamine Fluoride (SDF) with a tooth-coloured glass ionomer restorative material. SDF is first applied to suspected areas of dental decay to help arrest or slow the progression of the disease by reducing bacterial activity and hardening softened tooth structure. Following SDF treatment, a glass ionomer restoration may be placed to restore form and function, protect the tooth, and improve resident comfort. When appropriate, our team may also provide Interim Stabilization Treatment (IST) by placing a temporary restorative material to stabilize active cavities, preserve natural tooth structure, and support ongoing oral function. The primary goals of IST and SMART treatment are to: Reduce or eliminate tooth sensitivity and discomfort. Slow or arrest the progression of dental decay. Decrease the risk of pain, infection, and dental emergencies. Improve the resident's ability to eat and speak comfortably. Delay or prevent the need for more invasive dental procedures. Support overall oral health, comfort, and quality of life. These restorations and treatments can often remain functional for months or even years, depending on the size and location of the lesion, oral hygiene practices, dietary habits, and the resident's overall oral condition. While considered interim or minimally invasive in nature, they frequently provide significant long-term benefits for residents who may not be candidates for conventional restorative care. Residents receiving SMART, SDF, or IST treatment will continue to be monitored regularly by our dental hygiene team. Treated areas and restorations will be reassessed at subsequent visits and maintained, repaired, or referred for further treatment as clinically indicated. In many cases, these minimally invasive approaches allow residents to maintain pain-free oral function and comfort while receiving care in the familiar environment of their long-term care home, helping to preserve dignity, quality of life, and overall well-being.
